Placements24 is assisting a community services organization in Paarl, Western Cape, to recruit a Social Support Facilitator. You will work in the Paarl community, providing direct support, coordinating access to essential services, and guiding clients toward greater independence.

The role emphasizes empathy, resilience, and a commitment to social justice. A valid driver’s license and access to a reliable vehicle are required for fieldwork in the Western Cape.

Qualifications

• Relevant qualification in Social Work, Community Development, or related field.
• Proven experience (2–3 years) in social support, case management, or community outreach.
• In-depth knowledge of local social issues and Paarl resources.
• Excellent interpersonal, communication, and active listening skills.
• Empathy, patience, and non-judgmental client interaction.
• Ability to work independently in the community and manage your schedule.

Responsibilities

• Provide direct support and advocacy for individuals and families in need.
• Facilitate access to housing, employment, healthcare, and education services.
• Conduct needs assessments and develop personalized support plans.
• Organize and lead group activities, workshops, and support sessions.
• Offer emotional support and counseling to clients navigating difficult circumstances.
• Collaborate with local agencies, NGOs, and community leaders to coordinate services.
• Maintain confidential client records and document progress accurately.
• Monitor client well-being and report concerns promptly.
• Promote self-sufficiency and empowerment among clients.
• Participate in team meetings, case reviews, and professional development.
