Qualifications

A relevant qualification in Gerontology, Social Work, Nursing, or a related healthcare field.
Minimum of 3 years of experience working with the elderly, preferably in a care coordination or navigation role.
Thorough knowledge of geriatric care needs and resources in Durban, KwaZulu-Natal.
Excellent communication, interpersonal, and empathetic listening skills.
Strong organizational and time-management abilities.
Ability to work effectively independently and as part of a multidisciplinary team.
Proficiency in electronic health records (EHR) systems and standard office software.
Patience, compassion, and a genuine dedication to serving seniors.
Valid driver's license and reliable transportation.
Willingness to undergo background checks and provide professional references.

Responsibilities

Conduct comprehensive assessments of elderly clients' needs, including physical, social, and emotional well-being.
Develop personalized care plans in collaboration with clients, families, and healthcare professionals.
Coordinate and facilitate access to medical services, home care, and therapeutic programs.
Provide information and guidance on available community resources, such as senior centers and support groups.
Monitor client progress and adjust care plans as necessary.
Act as a liaison between clients, families, healthcare providers, and other service agencies.
Offer emotional support and companionship to clients and their families.
Educate families on elder care options and management strategies.
Maintain accurate and confidential client records and documentation.
Organize social activities and community engagement events for seniors.
