Qualifications

Responsibilities

• Conduct comprehensive assessments of elderly individuals' needs, including physical, emotional, and social well-being.
• Develop and implement tailored care plans, coordinating services such as home care, medical appointments, and social activities.
• Liaise with families, caregivers, doctors, and other healthcare professionals to ensure integrated care.
• Monitor progress and well-being of clients, adjusting care plans as needed.
• Provide emotional support and companionship to clients and their families.
• Assist clients in accessing community resources and support services.
• Manage schedules for caregivers and support staff, ensuring adequate coverage.
• Maintain accurate and confidential client records and documentation.
• Educate families on available resources and best practices in elderly care.
• Respond promptly and compassionately to client needs and emergencies.
