Placements24 is seeking a dedicated Social Work Case Manager to join our team in Paarl, Western Cape. The role focuses on assessing client needs, developing personalized care plans, and connecting individuals with housing, employment, and healthcare resources.
You will work collaboratively with professionals to empower clients toward self-sufficiency and improved well-being within the Western Cape region.
Qualifications
- Bachelor's degree in Social Work or related field.
- Registered with the South African Council for Social Service Professions (SACSSP).
- Proven experience in case management, social work, or community services.
- Strong understanding of social welfare systems and available community resources.
Responsibilities
- Conduct thorough assessments of client needs, circumstances, and eligibility for services.
- Develop, implement, and monitor individualized case plans in collaboration with clients.
- Connect clients with appropriate community resources, including housing, employment, and healthcare services.
- Provide ongoing support, advocacy, and counseling to clients throughout their case management journey.
- Maintain accurate and up-to-date case files, documentation, and progress reports.
- Liaise with relevant government agencies, non-profit organizations, and healthcare providers.
