Placements24 is seeking a Disability Support Officer to provide person-centred support to individuals with disabilities, helping them live more independently and participate in the community. Based in Centurion, Gauteng, the role focuses on developing and implementing personalised support plans and advocating for rights.

You will work with clients, families and health professionals, conduct risk assessments, organise community activities, and train support workers.

Qualifications

• A relevant qualification in Disability Studies, Social Work, Occupational Therapy, or a related field.
• A minimum of 4 years of direct support experience.
• In-depth knowledge of disability legislation and person-centred planning.
• Strong communication, interpersonal and empathy skills.

Responsabilités

• Develop, implement, and regularly review individualised support plans tailored to clients with disabilities.
• Provide direct support in daily living skills, personal care, community access and recreation.
• Facilitate client participation in social, educational and vocational activities within the Centurion community.
• Conduct risk assessments and implement safety measures for client well-being.
• Liaise with families, guardians, allied health professionals and service providers for coordinated care.
• Maintain confidential client records and progress notes.
• Advocate for clients' rights and promote independence and inclusion.
• Assist clients in navigating service systems and accessing resources.
• Organise and coordinate community-based programs and events to foster social connection.
• Provide training and guidance to support workers and volunteers.
