Formación

• Relevant qualification in Disability Services, Community Services, Allied Health, or a related field.
• Proven experience in disability support, case management, or coordination.
• In-depth knowledge of disability legislation, NDIS (or equivalent) processes, and available community resources.
• Exceptional organizational, time management, and problem-solving skills.
• Strong communication and interpersonal abilities, with a compassionate and client-focused approach.

Responsabilidades

• Develop, implement, and review individual support plans tailored to the specific needs and goals of each client.
• Coordinate a range of services, including healthcare, therapy, accommodation, and community participation.
• Act as a primary point of contact for clients, their families, and support networks, providing clear and consistent communication.
• Advocate for clients' rights and ensure their needs are met by service providers and government agencies.
• Conduct regular assessments of client progress and adjust support plans accordingly.
