Placements24 is seeking a dynamic Disability Support Program Facilitator in Tembisa, Gauteng. You will assess client needs, connect them to resources, and support service access to promote independence and inclusion.

In this on-site role, you will develop action plans, deliver workshops, build relationships with providers, and advocate for clients while maintaining confidential records. Travel within the area is required, and professional development opportunities are provided.

Qualifications

• A relevant degree in social work, community development, disability studies, or related field.
• 2+ years of experience in disability support, case management or community outreach.
• Knowledge of disability types, support services and SA legislation.

Responsibilities

About the Role

Our client is seeking a dynamic and empathetic Disability Support Program Facilitator to join their team in Tembisa, Gauteng . Key Responsibilities

• Assess the support needs of individuals with disabilities, developing personalized action plans.

Requirements

• A relevant qualification in social work, community development, disability studies, or a related field.
• A minimum of 2 years of experience in disability support, case management, or community outreach.
• In-depth knowledge of disability types, support services, and relevant legislation in South Africa .
• Strong interpersonal and communication skills, with the ability to build rapport with diverse individuals.
• Excellent organizational and time-management abilities.
• Proficiency in using standard office software and case management systems.
• A patient, empathetic, and non-judgmental approach.
• Ability to work both independently and collaboratively within a team environment.
• Willingness to travel within the Tembisa area and surrounding communities.
