Job summary

Placements24 partners with a leading Community & Social Care organization in Midrand, Gauteng, ZA to recruit a Community Support Worker. The role focuses on empowering vulnerable adults to live independently within their communities and includes home visits, activity facilitation, and coordination with care professionals.

Hybrid work model with in-person client interaction and flexible remote work is available.

Qualifications

• Relevant qualification in social work, community development, nursing, or related field is highly desirable.
• Proven experience with vulnerable adults, disabilities, mental health challenges, or age-related needs.
• Excellent communication, interpersonal and active listening skills.
• Strong organizational and time-management abilities, with caseload management capability.
• Proficiency in basic computer skills for record-keeping and reporting.
• A valid driver’s license and reliable vehicle access for community visits.

Responsibilities

• Conduct initial assessments of client needs and develop personalized support plans.
• Provide direct support with daily living activities (personal care, meals, tasks) per care plans.
• Facilitate community engagement and participation in activities.
• Monitor well-being, document progress, and report changes to supervisor.
• Administer medication as prescribed and maintain records.
• Liaise with families, healthcare professionals and other support services.
• Advocate for clients' rights, choices and dignity.
• Assist clients in developing life skills and independence.
• Maintain confidentiality and adhere to policies and procedures.
• Participate in supervision, team meetings and professional development.
