Job summary

Placements24 is seeking a compassionate Social Worker to deliver direct services, case management, and community advocacy in Rustenburg, North West. The role involves working with vulnerable populations, assessing needs, and connecting clients with vital resources.

You will conduct psychosocial assessments, develop care plans, and collaborate with agencies to ensure holistic client care within the Rustenburg region.

Qualifications

• Bachelor's or Master's degree in Social Work from an accredited institution.
• Registration with the South African Council for Social Service Professions (SACSSP) is mandatory.
• Experience in community-based settings is preferred.
• Knowledge of social welfare legislation and local resources in Rustenburg/North West.
• Excellent assessment, interviewing, counseling, and crisis intervention skills.
• Strong interpersonal and communication abilities with empathy and cultural sensitivity.
• Ability to manage a caseload and work under pressure.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify client needs and strengths.
• Develop and implement individualized care plans in collaboration with clients.
• Provide case management services, including resource navigation, referrals, and follow-up support.
• Offer counseling and emotional support to individuals and families facing challenges.
• Advocate for clients' rights and access to services within Rustenburg and beyond.
• Maintain accurate and confidential client records in compliance with policies and legal requirements.
• Collaborate with community agencies and stakeholders to ensure holistic client care.
• Participate in team meetings, case conferences, and professional development activities.
• Organize and facilitate support groups and workshops for clients and the wider community in North West.
