Placements24 is recruiting a Community Case Manager in Randburg, Gauteng. You will assess client needs, develop individualized care plans, and coordinate access to social and health services within the local community.

The role requires 3-5 years in case management, a relevant qualification in Social Work, Psychology, or Community Development, and strong communication and organizational skills. You will advocate for clients and maintain confidential records.

Qualifications

• 3–5 years of experience in case management or a similar community support role.
• In-depth knowledge of social services, healthcare systems, and community resources in Gauteng.
• Proven ability to assess needs, develop care plans, and coordinate services.
• Excellent communication, interpersonal, and problem-solving skills.
• Must be based in or near Randburg, Gauteng.

Responsibilities

• Conduct comprehensive assessments of client needs, circumstances, and eligibility for services.
• Develop, implement, and monitor individualized case management plans.
• Connect clients with appropriate community resources, healthcare providers, and social services.
• Advocate for clients' rights and ensure they receive timely and effective support.
• Maintain accurate, confidential, and up-to-date client records and case notes.
• Collaborate with other professionals, agencies, and stakeholders to ensure coordinated care.
• Provide ongoing support and follow-up to clients to ensure progress towards goals.
• Manage a caseload efficiently, prioritizing client needs and service delivery.
