Job summary

Placements24 is seeking a dedicated Case Manager to join our Randburg team, focusing on individuals recovering from mental health challenges or brain injuries. You will develop rehabilitation plans, coordinate services, and advocate for clients within the Randburg community.

The role emphasizes collaboration with families, healthcare professionals, and community agencies, driving client independence and well-being through individualized support and crisis intervention when needed.

Qualifications

• Minimum of 3 years of experience in case management within mental health or rehabilitation services.
• Relevant tertiary qualification in Social Work, Psychology, Occupational Therapy, or related field.

Responsibilities

• Conduct comprehensive assessments of clients' needs, strengths, and support requirements.
• Develop, implement, and monitor individualized rehabilitation and support plans.
• Coordinate access to housing, employment, healthcare, and social services.
• Act as liaison between clients, families, healthcare professionals, and community agencies.
• Provide ongoing support, counseling, and advocacy to help clients achieve personal goals.
• Facilitate crisis intervention and management as needed.
• Maintain confidential case records and progress reports.
• Collaborate with a multidisciplinary team to ensure holistic client care.
• Promote client independence, self-advocacy, and community participation.
