Job summary

Placements24 is seeking a dedicated Family Support Coordinator to join our Port Elizabeth team. You will work directly with families in the community, assessing needs and developing tailored support plans to connect them with housing, healthcare, and education resources.

The role requires hands-on engagement, strong communication, and the ability to manage a caseload while liaising with schools and social services.

Qualifications

• A relevant qualification in Social Work, Community Development, or a related field.
• Experience in family support, case management, or community outreach.
• Strong understanding of family dynamics and child development.
• Excellent communication, negotiation, and advocacy skills.
• Ability to work independently and manage a caseload effectively.
• Valid driver’s license and access to reliable transportation for community visits.

Responsibilities

• Conduct comprehensive assessments of family needs and strengths.
• Develop and implement individualized family support plans.
• Provide practical assistance and guidance to families to access community resources, such as housing, healthcare, and education.
• Facilitate parenting workshops and skill-building sessions.
• Liaise with schools, healthcare providers, and social services to coordinate support.
• Maintain detailed records of family progress and interventions.
