Caring Hand Home Health Solutions, Inc. seeks an experienced LPN/LVN for per diem home health nursing. The role involves providing direct patient care, following RN plans, and educating families to support patients’ recovery at home.

Per diem position with visits, requiring travel to patient homes. California licensure and CPR certification are mandatory; strong communication and teamwork are essential for success in a family-oriented environment.

Formación

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• Current CPR certification with AHA.

Responsabilidades

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.

Please note that this role is per diem with a rate of $45 - $55 per visit.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.

• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.
