Best Home Health Providers Inc. is seeking a licensed practical/vocational nurse to provide direct patient care in home health settings across California. You will implement individualized care plans, educate patients and families, and document care in a timely manner.

The role requires an active LPN/LVN license, CPR certification, and 3+ years of nursing experience. A reliable vehicle and driver’s license are required to travel to patient homes.

Formación

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsabilidades

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Coordinate hospice services — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $50 - $60 per visit.
