Premier Home Health Providers Inc. is seeking a per diem Licensed Practical Nurse (LPN/LVN) to provide direct patient care in home settings. You will implement care plans, educate families, and document care under supervision of an RN.

Travel to patients’ homes is required, and CPR certification plus a California license are essential. The role offers flexible scheduling with potential to convert to full-time based on performance, and benefits include mileage reimbursement and ongoing skills

Formación

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle; ability to travel to patient homes.

Responsabilidades

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely and adhere to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $45 - $55 per visit.

Responsibilities
