Best Home Health Providers Inc. is seeking a Medical Social Worker to lead and grow our social services team, guiding care for patients in home settings.

The role emphasizes supervision of social workers, program development, and ensuring high-quality documentation and compliance. The position is per diem with a rate of $100–$120 per visit, offering flexible scheduling and potential for full-time conversion based on performance.

Formación

• MSW from an accredited program.
• Active California license as an LCSW.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication skills and the ability to build trust with patients and families.
• A self-directed, organized, and flexible approach—you take initiative and work well with others.

Responsabilidades

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
