Caring Hand Home Health Solutions, Inc. is seeking a Medical Social Worker to lead our social services team in a per diem role. You will supervise social workers, coordinate care with clinical teams, and ensure compliance with state and ACHC standards.

The role requires an MSW from an accredited program and an active California LCSW license, with at least 3 years of management experience, ideally in home health. Strong communication and organization are essential.

Responsabilidades

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision and supporting professional growth.
• Oversee assessment, planning, and evaluation of social services care, ensuring documentation meets state and ACHC standards.
• Partner with the Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and identify new ways to better serve patients.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
