Job summary

SKIL Resource Center in Parsons, KS seeks a Youth Transition Coordinator to provide and coordinate Independent Living Services for youth transitioning to independent living. The role involves working with schools, families, and community agencies to empower youth and develop self-directed goals.

Qualifications

• At least 18 years of age.
• Have a valid Driver's License.
• Be familiar with Independent Living philosophy.
• Clear background checks.

Responsibilities

• Provide the five core services: Individual and Systems Advocacy, Independent Living Skills Training, Peer Counseling, Information and Referral and Deinstitutionalization.
• Assist youth with community resources such as Housing, Transportation, Jobs and other resources as needed.
• Develop and update Independent Living Plans with youth, focusing on strengths, interests and individual goal setting.
• Provide assistance in identifying and coordinating existing services that will assist in achieving the independent living goals established in the Independent Living Plan.
• Teach independent living skills and job readiness skills, such as effective communication, teamwork and problem-solving.
• Identify barriers to integration within the community and actively pursue the removal of those barriers through the Agency’s ADA Advocacy Implementation Plan.
• Work with youth’s support network team as needed when assisting youth in developing a person-centered plan and achieving desired goals.
• Empower youth to identify their own needs, gain problem-solving skills and advocate for themselves.
• Work as a liaison between schools, families, employers and agencies involved in helping the youth.
