Job summary

Southwest Network Company Brand is seeking a Transition Facilitator to coach and support teens and young adults aged 16.5 to 22 through transitions from youth to adult systems. You will develop tailored service plans, evaluate suitability for the program, and work with a transition team to overcome barriers across behavioral health, education, housing, and finances.

Occasional non-traditional hours and community-based work are required.

Qualifications

  • Minimum educational pathways include a Master's or Bachelor's in behavioral health or related field.
  • Alternative: Bachelor's in non-related field plus 1 year related experience.
  • Alternative: Associate's plus 1 year related experience.
  • Alternative: High school diploma plus 3 years related experience.

Responsibilities

  • Coach and support teens and young adults (16.5–22) through transitions.
  • Evaluate youth to determine program fit and develop service plans.
  • Collaborate with other service providers and educators across systems.
  • Work in the community with non-traditional hours and handle admin tasks.

Job description

If you are passionate about working with teens and young adults, this position could be right for you. As a Transition Facilitator, you will coach, support and help young adults (ages 16.5 to 22) with emotional, behavioral or mental health issues as they transition from the children’s behavioral health system of care into adulthood or to the adult system of care. You will provide guidance and support in the areas of employment, education, housing, social and family relationships, independent and daily living, finances and budgeting, decision making, emotional and physical health and wellbeing, community life, and more using principles of the nationally recognized Transition to Independence Process program.

You will engage as a member of a transition team to develop solution-focused strategies to overcome barriers. You will be responsible to evaluate youth to determine their suitability to enter the program, then work with them and facilitate service plans to progress them through the program until completion. You will work collaboratively with other service providers and educators from children’s mental health, education, adult mental health, child welfare, rehabilitation and other partners as needed to ensure appropriate support for each young adult. You will work in the community and at non-traditional hours, in the places and at the times that work best for these young adults. You will also be responsible for administrative and documentation functions.

REQUIREMENTS
Education, Certification, and Experience Requirements
  • Must possess one of the following:
    • Master's or bachelor's degree with a behavioral health or related field
    • Bachelor's degree in a non-related field PLUS one year related experience
    • Associate's degree in any field PLUS one year related experience
    • High school diploma or equivalency certificate PLUS three years' related experience
  • At least one (1) year of behavioral health experience facilitating Child and Family teams preferred.
  • Experience with transition-aged youth in the behavioral health, foster care and/or juvenile justice systems preferred.
  • System experience and understanding of service planning for seriously mentally ill adults desired.
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network).
  • Must maintain a valid Arizona state fingerprint clearance card.
  • Must maintain a valid Arizona driver's license and car insurance.
  • Must be at least 21 years of age.
