Job summary

Community Health Of South Florida Inc in Miami, FL seeks a Transition to Independence (TIP) Facilitator. This hourly, full-time role coaches youth ages 14-21 to achieve life goals through mentoring, teaching skills, counseling, and problem solving.

You will support up to 15 youths, document progress, and develop person-centered Transition Support Plans while collaborating with schools and service providers to ensure access to needed resources.

Qualifications

• Bachelor's degree in social sciences, rehabilitation, and/or education fields.
• At least 6 months experience in educational, social service, juvenile justice, or mental health settings.
• Must be 21+ years old and have a Florida driver's license.
• Maintain CPR certification from the American Heart Association.

Responsibilities

• Conduct strength-based needs assessments for youth ages 14-21.
• Coach up to 15 youths through service levels and record progress.
• Develop and document Transition Support Plans (ISP) responsive to each youth.
• Collaborate with teachers, counselors, and service providers to ensure needed services.
