Job summary

Stevens Treatment Programs in Fall River, MA is seeking a dedicated Case Manager for its Transitional Living Program (TLP). This active, relationship-based role blends individualized case management with hands-on work in the community to support adolescents and young adults toward independence.

You will coordinate services, help youths set goals, access housing and education resources, and document assessments and planning.

Qualifications

• High school diploma required; Bachelor’s degree preferred.
• Excellent engagement, communication, and organizational skills.
• Valid driver’s license and acceptable driving record.
• Personal vehicle with current, valid vehicle registration and insurance.
• Ability to complete required Residential Counselor and behavioral-support training, including physical intervention/restraint training when necessary.

Responsibilities

• Provide individualized case management and hands-on community work for adolescents and young adults.
• Coordinate services, referrals, and meetings between youth, families, DCF, schools, employers, and treatment team.
• Complete assessments and case management documentation and participate in treatment planning, transition and discharge planning, and aftercare.
