Stevens Treatment Programs is seeking a Case Manager for the Transitional Living Program in Fall River, MA to support adolescents and young adults as they prepare for independence, education, employment, housing, relationships, and community connections.
This active, relationship-based role combines individualized case management with hands-on work in the community, coordinating services, completing assessments, and contributing to treatment, transition, and aftercare planning.
Make a real impact as young people prepare for what comes next.
At Stevens Treatment Programs, we support adolescents and young adults as they build the skills, relationships, and confidence needed for greater independence. Our Transitional Living Program works with youth on the practical and personal pieces of moving toward adulthood - from education and employment to housing, relationships, community connections, and long-term stability.
As the TLP Case Manager, you'll support adolescents and young adults as they prepare for greater independence and a successful transition into adulthood. This is an active, relationship-based role that combines individualized case management with hands-on work in the community.
You'll help young people identify goals, build practical independent-living skills, navigate education and employment, access community resources, and address needs related to housing, transportation, health care, and other areas of adult life. You'll help youth strengthen connections with family members and other important people in their lives and support individualized life-skills and permanency planning.
You'll serve as an important link between youth, families, DCF, schools, employers, community providers, and the treatment team - coordinating services, appointments, referrals, meetings, and other supports. You'll also complete required assessments and case management documentation and play an active role in treatment planning, transition and discharge planning, and aftercare.

You're organized, dependable, and able to manage a lot of moving pieces without losing sight of the young person at the center of the work. You communicate well, build relationships naturally, and are comfortable working independently in the community as well as collaboratively with a multidisciplinary team.
This is an opportunity to do more than coordinate services. You'll help young people practice adulthood while they still have a team behind them - building skills, strengthening relationships, solving real-life problems, and creating a plan for what comes next.
