CenClear is hiring an Independent Living Case Manager to support youth in foster care as they transition to independent adulthood. You will develop individualized plans, coordinate with community partners, and empower youths by connecting them to education, job training, and tutoring resources.

Ideal candidates will have empathy, strong communication, and boundary-setting skills, plus a drive to help families and communities thrive.

Qualifications

• High school diploma is required.
• 2- or 4-year degree in therapeutic recreation or related field.
• Experience working with youth.
• Valid driver's license and reliable transportation for local travel.
• Flexibility to work some evenings.
• Willingness to obtain additional training as needed.
• Strong oral and written communication skills.
• Excellent organizational and multitasking abilities.
• Ability to network/collaborate with internal and external teams.
• Knowledge of treatment plans for adolescents and ability to work with adults and teens.

Responsibilities

• Travel within Clearfield County; mileage reimbursed per policy.
• Work 37.5 hours per week, 52 weeks per year.
• Build relationships with community partners and connect youth with resources.
• Set and monitor individual goals; conduct biannual assessments; lead sessions.
• Submit timely documentation and meet billable hour requirements.
• Coordinate with the Therapeutic Recreation team and plan with colleagues.
• Participate in training and workshops for skill development.
