Job summary

Community Mental Health Affiliates, Inc. (CMHA) is seeking a full-time Case Manager for the Transitional Youth Program in New Britain, CT. This 2nd shift role runs Sunday through Thursday, 3:00 pm to 11:00 pm, with a starting wage of $20.00 per hour.

The position focuses on supporting clients toward independent living and coordinating care across CMHA services. Responsibilities include maintaining caseloads, delivering education and life-skills training, assisting with finances and daily living

Qualifications

• Bachelor’s Degree in a mental health related field preferred or equivalent experience.
• Proficient with Microsoft Office (Word, Outlook, Excel, PowerPoint).
• Experience with Electronic Medical Record (EMR) systems and documentation.

Responsibilities

• Maintain caseload as determined by CMHA and funder.
• Provide direct care including education, skill-building, and supervision of activities.
• Teach life skills to help clients live independently.
• Develop and review individualized treatment plans with clients.
• Document progress notes, treatment plans, and required forms.
• Assist with medication administration if applicable and per policy.
• Coordinate with CMHA teams to ensure continuity of care and services.
• Possibly provide transportation as needed.
