Job summary

Community Mental Health Affiliates, Inc. (CMHA) is hiring for a full-time 2nd shift Respite Case Manager for our Residential IACTT Program located at 235 Beaver St., New Britain, CT 06051.

The role runs Monday–Friday, 3:30pm–11:30pm, supporting clients toward independent living and coordinating care within CMHA’s comprehensive services. Compensation starts at $20.00 per hour with a benefits package including health insurance, retirement plan, holidays, PTO, EAP, tuition discounts, and ongoing

Responsibilities

• Maintains appropriate caseload as determined by CMHA and funder.
• Provides direct care to clients through individualized and group activities, monitoring safety and environment.
• Teaches life skills to help clients live independently, including finances, daily living, housing, and nutrition guidance.
• Educates clients to access emergency and community resources.
• Develops and reviews individualized treatment plans with clients.
• Ensures timely documentation per CMHA policies (progress notes, treatment plans, authorizations).
• Supervises self-administration of medication when applicable and documents accordingly.
• Collaborates with CMHA teams to coordinate services and communication with treatment teams.
• Communicates with external providers to ensure access to needed services; may provide transportation.
• Performs other related duties as assigned
