Job summary

Community Mental Health Affiliates, Inc. in New Britain, CT is seeking a Case Manager for the Transitional Youth Program (TYP). This full-time role operates Sunday through Thursday, 3:00 pm – 11:00 pm, located at 233 Main St., New Britain.

The position starts at $20.00 per hour with strong emphasis on client safety and service coordination. Responsibilities include caseload management, delivering education and life-skills support, developing treatment plans, and collaborating with the CMHA team

Qualifications

• Bachelor’s degree preferred or equivalent experience.
• Experience in case management or related field.
• Proficiency with Microsoft Office.

Responsibilities

• Maintain appropriate caseload and provide direct client care.
• Develop individualized treatment plans and monitor progress.
• Coordinate care with CMHA teams and external providers.
• Document in EMR and ensure confidentiality.
