Transitional Residence for Youth in Topeka, KS is seeking a compassionate Case Coordinator to guide young residents through critical transitions, connecting them with resources and supports. You will develop individualized plans and monitor progress to ensure safe, stable outcomes.

This role emphasizes collaboration with staff, families, and community providers, delivering life skills workshops and advocating for youth needs within the organization.

Qualifications

• Bachelor's degree preferred in social services or related field.
• Experience in case management or youth services highly valued.
• Knowledge of trauma-informed care and youth development principles.

Responsibilities

• Develop, implement, and monitor individualized case plans for youth residents
• Coordinate services and referrals to community resources, including mental health, education, and employment support
• Conduct regular check-ins and assessments to track client progress and adjust care plans as needed
• Maintain accurate and up-to-date case documentation and client records in compliance with program standards
• Collaborate with internal staff, external agencies, and family members to ensure holistic support for each youth
• Facilitate life skills workshops and group sessions to promote independence and self-sufficiency
• Advocate for youth needs within the organization and with outside service providers
