Job summary

Rite of Passage Lake Granbury Youth Services in Granbury, TX is hiring a Case Manager to join the site’s Clinical team. You will develop and facilitate case plans for youth, coordinate court hearings, and liaise with families, staff, and agencies.

Qualifications include a Bachelor’s degree, 21+ age, and ability to pass background checks and TB tests. This is a full-time, on-site position with strong growth opportunities and a comprehensive benefits package.

Qualifications

• Bachelor's degree required; 21+ years of age; able to pass background checks, drug, TB tests.
• Must meet requirements to become an ROP eligible driver with a valid license.

Responsibilities

• Develop and implement individualized case plans and treatment programs for youth.
• Provide progress notes and coordinate court hearings, probation visits, home passes and official visits.
• Serve as primary liaison among student, family, staff, and placing agencies.
