Job summary

Rite of Passage is hiring a Case Manager for Lake Granbury Youth Services in Granbury, TX. The role involves managing case plans, coordinating with families, staff, and placing agencies, and documenting progress while coordinating court hearings and supervised visits.

You will work within a trauma-informed, youth-centered program that supports positive development and safe, therapeutic services.

Qualifications

• Possess a Bachelor's Degree in a relevant field.
• Be at least 21 years of age.
• Pass a criminal background check, drug, physical, and TB tests.
• Pass a search of the child and adult abuse registries.
• Meet requirements to become an ROP eligible driver with a valid Driver's License.

Responsibilities

• Administer, develop, and facilitate the completion of case plans and treatment programs.
• Serve as primary liaison between student, parents/guardians, staff, and placing agency officials.
• Provide progress notes and organize court hearings, Probation Officer visits, student visits, and home passes.
• Support mission-driven care at Lake Granbury Youth Services in Granbury, TX.
