Job summary

Rite of Passage seeks a Case Manager for Canyon State Academy in Queen Creek, AZ. The role focuses on administering and coordinating case plans and treatment programs for youth within a state-approved program on a large campus.

Qualified candidates have a BA/BS in a related field and at least 3 years of case management experience with Youth/Juvenile placements. A background check, drug screen, medical exam and TB test are required.

Qualifications

• BA/BS in a related field.
• 3 years of case management experience with Youth/Juvenile placement agencies.
• Must be at least 21 years old.
• Ability to pass a criminal background check, drug screen, physical exam, and TB test.

Responsibilities

• Administer, develop and facilitate completion of case plans and treatment programs as requested by the placing agency and student needs.
• Report to the Director of Student Services / Case Management Coordinator depending on location.
