Job summary

Apalachee Center, Inc. provides comprehensive behavioral health services across 8 counties in North Florida. This role engages in assessing youth, conducting home visits, and coordinating service plans to support treatment and family needs.

Ideal candidates hold a related Bachelor's degree, a valid driver's license, and related case management credentials; experience with youth with serious mental illness is preferred. This is a full-time on-site position with competitive benefits.

Qualifications

• Bachelor's degree in a related human services field is required.
• Current valid driver’s license is required.
• ADM Targeted Case Manager Certification preferred or to be maintained.

Responsibilities

• Conduct comprehensive assessments of youth strengths and needs.
• Perform home visits to evaluate safety and well‑being prior to intake.
• Develop individualized service plans with measurable goals for youth and families.
• Coordinate with providers across systems to ensure access to needed services.
• Document services and progress in the client record within required timelines.
