Job summary

Qualifications

• Bachelor's degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, or related human services field preferred.
• Current valid driver’s license.

Responsibilities

• Assists clients in gaining access to financial, insurance, employment, medical, social, education, and other services.
• Completes assessments and develops service plans with clients and significant others.
• Meets service productivity standards for case management and maintains proper documentation.
• Coordinates care between service providers, CSU, and other required services.
• Documents services in client records with clear linkage to goals and progress.
• Identifies high-risk clients and completes suicide risk assessments as needed.
• Attends and participates in staff/unit meetings.
