Qualifications

• CBHCM certification verified through the Florida Certification Board (FCB).
• Bachelor’s degree in counseling, social work, psychology, nursing, or related human services field.

Responsibilities

• Assessments: conduct initial and ongoing assessments and documentation; ensure within required timeframes.
• Home visits: perform home visits to assess safety and well-being; document findings.
• Service plans: develop individualized service plans with measurable goals and update them regularly.
• Case management: help clients access financial, medical, and social services; coordinate with supports.
• Service provision: meet productivity standards and ensure services align with care plans.
• Progress notes: document services in client records with timely updates and clear linkage to goals.
