Job summary

Apalachee Center, Inc. seeks a Certified Behavioral Health Case Manager to deliver targeted case management for adults with serious mental illness in the North Florida region.

You will conduct comprehensive assessments, make home visits, and develop individualized service plans with measurable goals in partnership with clients and guardians. Role requires a CBHCM or progression toward certification, a valid driver’s license, and experience coordinating access to benefits, employment, and

Qualifications

• Bachelor’s degree in a human services field is required.
• CBHCM certification or ability to obtain CBHCM within 12 months.
• Valid driver’s license and ability to drive as needed.
• Experience with targeted case management for adults with mental health issues preferred.

Responsibilities

• Assessments: perform preliminary bio-psychosocials, psychosocials, intake assessments, and medical questionnaires within required timelines.
• Home visits to assess safety and well‑being, with documentation in Avatar within 24 hours.
• Develop individualized service plans with measurable goals and update at least every 6 months.
• Assist clients in obtaining benefits, employment, housing, and education services.
• Maintain 85% compliance with service, documentation, and CSP requirements.
• Document progress notes detailing linkage to goals and client progress.
