Professional Services Group, Inc. is hiring a Care Coordinator for the CCS/CLTS Dual Care Coordination program in Milwaukee County. The role focuses on wraparound service delivery to create multi-disciplinary treatment plans addressing family strengths and needs.

The Care Coordinator will provide case management for 12–13 clients, engage families, and collaborate with schools, health professionals, and CFSS workers while handling crisis planning and safe transport as required.

Qualifications

• Bachelor's degree required in sociology, social work, psychology, or closely related field.
• Reliable transportation daily with valid driver’s license and automobile insurance.

Responsibilities

• Provide case management services to a caseload of 12-13 clients.
• Interview and evaluate clients and families during intake and assessment to analyze needs and strengths.
• Create and implement goal-oriented, strengths-based treatment plans.
• Facilitate team meetings with client, family, and providers to monitor progress.
• Develop crisis/safety plans for clients and families.
• Develop a family support team from formal and informal supports.
• Collaborate with school personnel, mental health professionals, CFSS workers, and court officials.
• Advocate for clients in emergency and crisis situations.

Job Summary

We are seeking compassionate and dedicated professionals to join our team as Care Coordinators in our Youth Dual Care Coordination program. This program supports children and their families in Milwaukee County who are facing challenges with severe mental health and/or substance use disorders, with co-occurring developmental and/or physical disability needs. Our programming follows the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both the strengths and needs of the family.

Key Responsibilities Of The Care Coordinator

• Provide case management services to a caseload of 12-13 clients.
• Interview and evaluate clients and families during the intake and assessment process to analyze client and family needs and strengths.
• Create and implement goal-oriented and strengths-based treatment plans.
• Facilitate team meetings with the client, family, and direct service providers to support and monitor progress of goals and services.
• Develop a crisis/safety plan for the client and family.
• Develop a family support team comprised of formal and informal supports.
• Work in collaboration with other providers, school personnel, mental health professionals, Child and Family Services workers, and court officials.
• Exercise judgement to intervene as an advocate for clients in emergency and crisis situations.
• Safely transport clients as needed.

LOCATION:

West Allis, WI. Local travel is required to meet with clients within their home and other community spaces throughout Milwaukee County and surrounding areas, with mileage reimbursement.

SCHEDULE:

Monday-Friday 8:00am-5:00pm, with occasional evenings to accommodate families' schedules.

PAY RATE:

Candidates with a Bachelor's Degree: $24.00-26.00 per hour

Candidates with a Master's Degree: $27.00-30.00 per hour
