Job summary

PSG/CIP in Wisconsin is hiring a Care Coordinator to join the Adult CCS Care Coordination program in Milwaukee County. The role focuses on wraparound, recovery-oriented, person-centered services for individuals with mental health and substance use challenges, delivering support in homes and communities.

The position offers typical hours Monday–Friday, 9:00 AM–5:00 PM with hybrid/remote scheduling. Salary ranges for Bachelor's and Master's degree holders begin at $23.00 and $26.00 per hour,

Qualifications

• Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related field (required)
• Valid driver's license, reliable daily transportation, and insurance
• 1+ year experience working with individuals with mental health or substance use needs (preferred)

Responsibilities

• Conduct comprehensive strengths and needs assessments during intake and throughout services
• Develop and implement individualized Recovery Plans of Care
• Coordinate and lead team meetings with service providers and support networks
• Provide crisis intervention and safety planning as needed
• Attend court hearings, psychiatry appointments, and provide advocacy support
• Safely transport clients to appointments when necessary
• Build strong formal and informal support teams

Position Overview

We are seeking a compassionate and driven Care Coordinator to join the Adult Comprehensive Community Services (CCS) Care Coordination program with Professional Services Group in Milwaukee County. In the Care Coordinator position, you will support individuals with significant mental health and substance use challenges. You'll deliver wraparound, strength-based services that empower clients in their homes and communities, using a recovery-oriented and person-centered approach.

Key Responsibilities
