Job summary

Center for Human Development, Inc. (CHD) in Springfield, MA is seeking a proactive Youth Service Coordinator to support youth transitioning from residential placements to the community.

You will work closely with CHD staff and DYS caseworkers to guide, monitor, and support youths as they access services and engage in pro-social activities at the District Office. The role focuses on reentry planning, resource navigation, transportation, and data entry while maintaining a safe, structured

Qualifications

• Bachelor’s degree in a related human services or criminal justice field preferred.
• Minimum of 2 years’ experience working with youth, ideally within juvenile justice or related settings.
• Valid driver’s license with clean driving record; willing to drive a 15-passenger van.

Responsibilities

• Attend discharge meetings (90-60-30 day) and assist youth transitioning back to the community.
• Coordinate with DYS caseworkers to connect youth with housing, benefits, tutoring, job readiness and mental health services.
• Accompany youth for basic needs shopping and transport in program vehicles; plan outings and activities.
• Provide office coverage, safety checks, attendance tracking, and maintain a welcoming environment.
• Enter client data into the JJEMS database and support program administrative tasks.
