Seven Hills Foundation in Massachusetts seeks a Youth Service Coordinator to support young people transitioning back to family, school, and community. You will work directly with youth involved with the Massachusetts DYS, building trust and providing consistent, trauma-informed support in homes, schools, and community settings.

You’ll develop individualized plans, connect youth with resources, advocate for their needs, and collaborate with families, DYS staff, schools, and partners to help youth

Qualifications

• Experience in case management, youth services, behavioral health, or related human services.
• Strong relationship-building and communication skills with youth and families.
• Ability to engage youth facing transitions or barriers to success.
• Advocacy, assessment, and service coordination abilities.
• Ability to collaborate with families, DYS, schools, and community partners.

Responsibilities

• Meet with youth in homes, schools, and communities to build trust.
• Develop individualized transition plans and connect youth with resources.
• Provide crisis support and maintain consistent documentation.
• Advocate for youth and coordinate with partners to remove barriers.
