Zusammenfassung

The Center for Human Development (CHD) seeks a proactive Youth Service Coordinator for the Community Services Network in Springfield, MA. This on-site, full-time role partners with DYS staff to support youths transitioning from residential placements back into communities.

Responsibilities include discharge planning, resource navigation, participant transport, and data tracking in JJEMS. Requires a Bachelor’s degree and 2 years’ youth experience, plus a valid driver’s license.

Qualifikationen

• Bachelor’s degree in a related human services or criminal justice field preferred.
• Minimum of 2 years of experience working with youth, preferably in juvenile justice, behavioral health, or child welfare.
• Valid Driver’s License, clean driving record, reliable personal vehicle; willingness to drive a 15-passenger van.

Aufgaben

• Attend discharge meetings and assist youth with transition planning; help obtain vital documents and open bank accounts.
• Coordinate with DYS caseworkers to connect youth with community resources and services.
• Accompany youth to purchase basic needs; transport them in program vehicles; lead recreational, educational, vocational, and civic outings.
• Maintain safe, organized district office operations; monitor attendance, curfews, GPS requirements, and security.
