Job summary

Correctional Healthcare Partners (CHP Health) is seeking a Discharge Planner to coordinate pre-release care management for youth in juvenile detention, aligning with CalAIM Justice-Involved services. You will build individualized transition plans and ensure continuity of medical, behavioral health, and social supports prior to release.

Qualifications

• Bachelor’s degree in a related field (e.g., Social Work, Psychology, Public Health).
• Minimum of two years in case management, care coordination, or discharge planning.
• Experience with adolescents and transitional-age youth.
• Knowledge of Medi-Cal, CalAIM, ECM, and Justice-Involved Initiative services preferred.
• Valid California driver's license and ability to travel locally.

Responsibilities

• Coordinate comprehensive pre-release care management services for justice-involved youth.
• Develop transition and care plans for community reintegration.
• Coordinate pre-release services in line with CalAIM Justice-Involved Initiative.
• Ensure continuity of health and behavioral health services after release.
• Collaborate with facility staff, plans, providers, and family members.
