Job summary

NaphCare, Inc. is seeking a Discharge Planner to join the Fulton County Jail team in Atlanta, GA.

You will assess medical, mental health, and social needs to craft post-release plans and coordinate with healthcare providers, social workers, and community agencies for smooth transitions back into the community. The role emphasizes documentation, medication management planning, and helping inmates identify support networks and access to services, including Apple Health enrollment assistance where

Qualifications

• Associate’s degree in healthcare field required.
• At least 1 year of clinical experience; correctional healthcare preferred.
• Active BLS CPR certification required.
• Experience with Medicaid and 1115 waivers preferred.
• Ability to maintain confidentiality and strong communication skills.
• Must pass facility background check and drug screening.

Responsibilities

• Perform discharge planning for all inmates released from the facility.
• Create individualized post-release plans including medical follow-up, medications, and social support.
• Collaborate with healthcare professionals, social workers, and community resources.
• Maintain accurate records of discharge plans and follow-up activities.
• Provide information to inmates and families about discharge options and available resources.
• Act as liaison between the facility and community services for referrals and access to care.
• Assist with medication management planning and identifying support networks.
• Coordinate with Apple Health navigators and assist with enrollment processes.
