Telecare Corporation is seeking a Discharge Planner in California to provide discharge planning services to clients and families. You will coordinate with the Conservator's Office, eligibility workers, and mental health services to ensure smooth transitions.

The role requires two years of related experience and certifications in CPR, CPI, and First Aid. Starting wage is $24.18–$29.88 per hour, with geographic differentials applied by location, and full-time daytime shifts from 8:00 AM to 4:30

Qualifications

• Two years of professional experience in Social Work, Nursing or a related field.
• Must be CPR, CPI, and First Aid certified on date of employment or within 60 days.
• All opportunities require post-offer physicals, background checks, licenses verification, and driving eligibility if applicable.

Responsibilities

• Provide discharge planning services to clients and families.
