Wexford Health Sources, Inc. is seeking a part-time Discharge Planner at the Criminal Justice Center in Memphis, TN.

The role focuses on coordinating discharge planning and continuity of care for chronic and high-need patients preparing for community release within a correctional setting. You will collaborate with inter-disciplinary teams, engage community resources, and ensure timely appointments, medication continuity, and data reporting to the health system.

Qualifications

• Education requirement: Graduate from an accredited School of Social Work, Case Management, Health Care Management or other related fields.
• Two (2) years of case management or related experience preferred.
• Licensing and Certification: None required.

Responsibilities

• Coordinate discharge planning and continuity of care for high-need patients upon release from a correctional facility.
• Interact with facility health care unit to assess needs and barriers to release.
• Prioritize and schedule follow-up appointments and ensure medication continuity post-release.
