Wellpath / Correct Care Solutions in California seeks an RN Discharge Planner to coordinate daily clinical reviews, utilization management, and care transitions for CalAIM-eligible individuals. You will collaborate with medical directors, providers, and community partners to ensure safe discharge planning and continuity of care.
This role requires an active RN license, 2 years of discharge coordination experience, and prior correctional/detention facility exposure.

The RN Discharge Planner H coordinates daily clinical care, utilization management, and care management activities while ensuring compliance with nursing standards and regulatory requirements. This role provides clinical oversight of the discharge planning process for CalAIM-eligible individuals, reviewing and approving reentry care plans to ensure they are clinically appropriate and aligned with Enhanced Care Management and Community Supports. The position conducts assessments, leads care coordination with managed care plans and community providers, and supports continuity of care. As the accountable clinician for discharge planning, this role helps facilitate safe and effective transitions of care.

The ideal candidate will have strong care coordination, discharge planning, and case management skills, with the ability to assess complex medical and behavioral health needs and develop effective transition-of-care plans. Candidates should be knowledgeable about Medi-Cal, CalAIM, community healthcare resources, medication coordination, and referral processes. Strong communication, documentation, critical-thinking, and interdisciplinary collaboration skills are essential, along with the ability to work effectively with justice-involved patients and community partners.
