Qualifications

• High School Diploma with two years’ experience working with at-risk youth.
• Bachelor’s degree in a social service or related field.

Responsibilities

• Conduct initial intake with client and family/guardian.
• Ensure completion of intake paperwork, assessments and ISP per policy.
• Maintain client files and documentation in SWK systems.
• Engage with community partners and promote SWK programs.
• Coordinate YAC youth involvement in Youth Summit and activities.
• Provide and coordinate post high school readiness activities.
