Job summary

Southwest-Key-Programs seeks a Service Coordinator/Group Facilitator for HHSC Dallas CYD in Dallas, TX. This part-time role delivers direct services to youth and families, facilitating group and recreational sessions per the Community Youth Development Program.

The position requires an Associate or Bachelor in social services, 4 years (or 2 years with a Bachelor’s) of youth-facing experience, bilingual English/Spanish, and flexibility including evenings, weekends, and on-call coverage.

Qualifications

• Education requirements: Associate's degree in social services or related field, or Bachelor’s degree in social services or related field.
• Four years of experience with youth and families with an Associate’s degree; or two years with a Bachelor's degree.
• Bilingual English/Spanish required.

Responsibilities

• Conduct initial intake with client and family/guardian.
• Create, track & organize physical files for assigned workload.
• Ensure timely completion of documentation (intake, session notes, surveys, dismissals) in SWK policy.
• Enter documentation in ETO and PEIRS after each session.
• Run reports in ETO to track clients and services.
• Keep consistent communication with families in assigned list.
• Plan and create lesson plans to facilitate weekly group sessions.
• Ensure curriculum is delivered professionally to meet learning objectives.
• Maintain sign-in sheets for all clients attending sessions.
• Facilitate and/or co-facilitate group sessions and recreation sessions.
• Provide advocacy, referrals and linkages to participants and families.
• Collaborate with school staff and community agencies for service delivery.
• Attend outreach events and promote program offerings.
• Recruit and retain participants in programming.
