Job summary

Pathways Navigator, Pathways Youth Navigation Center in Modesto, CA, seeks an experienced advocate to provide initial screening and ongoing service navigation for homeless youth aged 18-24. You will help develop individualized plans, coordinate services, and document in HMIS and related systems.

The role requires strong communication, Spanish language ability, and a commitment to trauma-informed care, harm reduction, and ending homelessness. Full-time, day shift.

Qualifications

• 1+ years’ experience in a related social services field preferred.
• Bilingual in Spanish is highly preferred.
• Experience working with youth 18–24 in supportive coaching.
• Knowledge of Positive Youth Development, Trauma-informed Care and Harm Reduction.
• Ability to use HMIS, Word and Excel for case management.

Responsibilities

• Conduct screenings and intakes for new participants.
• Develop individualized service plans with youth.
• Coordinate appointments, linkages, and aftercare activities.
• Maintain client intake documentation and HMIS data entry.
• Support Coordinated Entry meetings and partnerships with providers.
• Provide crisis prevention and safety planning as needed.
• Assist with data reporting and program development.
