Job summary

18 Degrees in Pittsfield, MA seeks an Outreach Case Worker to support youth ages 10-24 within the PCC, focusing on welfare, goals, strengths and progress tracking.

You will build trusting relationships, coordinate wraparound services with counseling and mentoring staff, provide referrals and advocacy, and help youth succeed.

Strong computer skills and relevant experience or a degree in human services preferred; bilingual/multicultural fluency is a plus.

Qualifications

• 4 years of relevant experience or Bachelor’s degree in a human service field preferred.
• Outreach and case management experience strongly preferred.
• Data collection experience a plus.
• Strong computer and internet skills, including Microsoft Office.
• Lived experience; multilingual/multicultural fluency are a plus.

Responsibilities

• Identify goals, strengths and tracking progress; coordinate needed services; provide warm referrals, individualized support & advocacy; and facilitate groups.
• Coordinate with counseling and mentoring personnel to facilitate multi-systemic wraparound services for at-risk youth (ages 10-24) within the city of Pittsfield, MA.
