Qualifications

• Knowledge of Eastern Panhandle Social Services networks.

Responsibilities

• Conduct screenings and intakes.
• Engage in regular outreach to homeless youth.
• Analyze youth needs and resources.
• Complete needs assessments for all eligible youth.
• Assist young adults in developing goals and objectives to formulate successful action plans.
• Provide case management services including advocacy, referrals, etc.
• Locating and referring clients to safe and affordable housing.
• Maintain accurate and timely case records, reports, and statistical data.
• Interact with low-income populations.
• Maintain confidentiality within and outside of the agency.
• Work independently using initiative, creativity and flexibility in meeting participant needs while preserving their dignity.

Job Summary: The Youth Outreach Case Manager provides a holistic approach to case management services and providing outreach to program participants who are disconnected youth ages 18-24 experiencing homelessness. This position is responsible for the identification of and outreach to homeless youth in Eastern Panhandle. The Youth Outreach Case Manager will interact with participants, community resources, staff members, volunteers, and other agency representatives. The position also includes coordination of appropriate community services to facilitate client success.

Essential Functions:

• Conduct screenings and intakes.
• Engage in regular outreach to homeless youth.
• Analyze youth needs and resources.
• Complete needs assessments for all eligible youth.
• Assist young adults in developing goals and objectives to formulate successful action plans.
• Provide case management services including advocacy, referrals, etc.

Qualifications: Minimum Requirements:

• Associate degree in Human Services and 1-2 years experience working with youth/young adults required.
• Additional experience may be used as a substitute for educational requirement.
• Valid drivers license and access to a vehicle on a continual basis.
• Experience providing holistic case management services to youth/young adults.
• Experience interacting with volunteers.
• Knowledge of Eastern Panhandle Social Services networks.
