Job summary

Qualifications

Responsibilities

• Provide ongoing monitoring of consumers and document observations timely.
• Engage consumers to ensure safety, participation, and needs are met.
• Assist with personal care, appointments, chores, and activities.
• Lead groups and activities per treatment goals and schedules.
• Collaborate with nursing, therapy, and recreation staff for milieu management.
• Respond to crises with proper interventions and de-escalation.
• Follow policies, maintain privacy, and report issues promptly.
• Support supervision and participate in performance improvement activities.
